The OSEP-funded Tracking, Referral and Assessment Center for Excellence (TRACE) at the Orelena Hawks Puckett Institute, in collaboration with the American Academy of Pediatrics (AAP), developed an early intervention referral form that physicians and other primary referral sources can use to make a referral to Part C early intervention programs. The AAP recently included this form in a revised policy statement entitled The Role of the Medical Home in Family-Centered Early Intervention Services. The AAP has also developed a computerized version of the referral form.

The computerized version of the referral form is available at http://www.medicalhomeinfo.org/health/EI.html
The policy statement is available at http://aappolicy.aappublications.org/cgi/content/full/pediatrics;120/5/1153
Guidelines for early intervention programs wishing to custom the referral form can be found at http://www.tracecenter.info/practiceguides/practiceguides_vol1_no2.pdf
