Connecticut General Supervision Activities for Part C to B Transition: FAPE by Three

The Connecticut State Department of Education (CSDE), Bureau of School and District Improvement has developed a general supervision process for those school districts that are identified as not providing FAPE to one or more children by age three as a result of a data merge related to transition between Part C and preschool special education.

• Connecticut General Supervision Activities 2006-2007 Part C to Part B Transition: FAPE by 3 (DOC: 192kb) - This document describes that general supervision activities that CDSE uses when an LEA has been identified as not providing FAPE by age three to one or more children. The activities include data verification, data review and analysis, optional on-site visit, feedback to district, improvement planning, and closure.
• Connecticut School District Data Verification Table for FAPE by 3 (DOC: 30kb) - This table must be completed by each school district for each child who was identified as not receiving FAPE by age three along with requiring additional information to be submitted to the CDSE.
• Sample Connecticut Letter to School District Requesting Completion of Transition Data Verification Table (DOC: 168kb) - This letter is a sample of the communication from CSDE sent to each school district requiring the completion of the Data Verification Table for FAPE by 3.
• Sample Connecticut Letter to School District Following Analysis of FAPE by 3 Data (DOC: 177kb) - This is a sample letter sent to an individual school district after review and analysis of the transition data verification table. The letter clarifies FAPE by 3 requirements and provides trend data on Connecticut's performance on this indicator.
